⚽ BainsProAcademy Injury, Insurance & Liability Waiver

By submitting this form, I acknowledge and agree to the following terms:

1. Assumption of Risk:  I understand that participation in soccer training, fitness, and conditioning activities involves inherent risks, including but not limited to sprains, fractures, concussions, and other injuries. I voluntarily assume all such risks for myself/my child.

2. Release of Liability:  I hereby release and hold harmless BainsProAcademy, its owners, coaches, contractors, employees, and affiliates from any and all liability, claims, or demands arising from participation in any BainsProAcademy program, training, or event — whether occurring on or off-site.

3. Medical Authorization:  In the event of injury or emergency, I authorize BainsProAcademy staff to obtain medical treatment for my child and agree to be financially responsible for any related expenses.

4. Insurance Responsibility:  I confirm that I carry personal medical and accident insurance that covers my child during participation in all BainsProAcademy activities. I understand that BainsProAcademy maintains general and coaching liability insurance but does not provide individual medical coverage.

5. Health Disclosure:  I have disclosed any relevant medical conditions, injuries, or limitations that may affect my child’s participation. I agree to notify the coach of any new injuries or health concerns prior to training.

6. Standards of Conduct & Safety: 

All sessions follow Strength & Conditioning (S&C) and player-safety standards. I agree that my child will comply with all instructions and conduct expectations to maintain a safe training environment.

7. Photo & Media Release: 

I grant permission for BainsProAcademy to use photos and videos of my child taken during training for marketing, educational, and social media purposes.
